Tayfun It looks like the prism blocked part of the imaging area.
You can loosen these three screws, and adjust the prism to align the long side of the sensor, if the three claws just in the way, you can add a spacer to change its position to try.
Regard of the focus, can you please take a picture of how you connect the camera and OAG? Thanks.
